USMLE Step 1 (more commonly just Step 1 or colloquially, Board ) is the first part of the US Medical License Examination. It aims to assess whether medical school students or graduates can apply important concepts of basic science that are fundamental to the practice of medicine. US medical students, as well as non-US medical students seeking medical licenses in the US, usually take Step 1 at the end of the second year of medical school. Graduates of international medical schools (ie, those outside the US or Canada) should also take Step 1 if they wish to practice in the US. Graduates from international medical schools must register through ECFMG, and the admission fee is $ 895. For 2016, the NBME enrollment fee for the exam is $ 600, with an additional fee for applicants who choose a test area outside the United States or Canada.

Prior to 1992, the NBME Part I exam served as an important basic science exam for medical students at the end of their second year. Following the launch of the three-part US Medical Consent Examination, the Part I NBME exam was continued in its new format, the USMLE Step 1 exam, which has since evolved into a clinically increasingly applied study of the basic sciences. The test became a computer based on a few years later. In May 2015, USMLE began to emphasize the concept of patient safety and quality improvement in all parts of the USMLE exam series, including Step 1.

While Step 1 USMLE has traditionally been taken after students complete basic science and before core clinical outings, in recent years a number of medical schools have moved to ask students to take USMLE Step 1 after clinical core clerkships.

Format

The current test is an eight hour computer-based test taken in a day, consisting of seven sections 40 questions with a maximum of 280 multiple choice questions. One hour is provided for each section, allocating an average of one minute and thirty seconds to answer each question. Among the test sections, test takers were given a cumulative time of 45 minutes (during the test day) for a private break. (There is a 15 minute tutorial at the beginning of the exam, which can be selected by examinees to pass and added time for rest periods.) If the taker completes each section before the allocated one hour deadline, unused time is added to the total rest time. This test is performed on one of several Prometric computer testing sites.

Subject

Step 1 is designed to test the knowledge learned during the medical science elementary years as applied in the form of a clinical sketch. These include anatomy, behavioral science, biochemistry, microbiology, pathology, pharmacology, and physiology, as well as interdisciplinary fields including genetics, aging, immunology, nutrition, and molecular and cell biology. Epidemiology, medical ethics and questions about empathy are also emphasized. Each test is dynamically generated for each test taker; while the proportion of common questions originating from a particular subject is the same, some test participants report that a particular subject is emphasized or justified.

Scoring

Currently, students receive a three-digit score after taking the Step 1 exam. In 1999, USMLE suspended the use of percentile-based systems to support a three-digit and two-digit scoring system. In October 2011, double-digit scores were no longer reported to any parties other than examinees. In April 2013, a two digit score is completely removed from the score report.

Although the USMLE program does not reveal how a three-digit score is calculated, Step 1 scores theoretically range from 1 to 300, most exam scores are in the range of 140 to 260, the graduation score is 194 and the national average and standard deviation are respectively around 229 and 20. Increase in the national average value Step 1 has been observed with time, as shown in the table below. According to the National Population Matching Program, the average value for the Allopathic US seniors matched to the residency program in 2016 was 233.2 (sd = 17.4).

Effects on residency match

The USMLE score is one of many factors considered by the residency program in selecting applicants. However, at this time, this test is only the standard size of all applicants. USMLE median value Step 1 for US Medical School graduates for various residencies is mapped in Chart 6 on page 9 of "Charting Outcomes in the Match" available through the NRMP Web Site

Many residency programs use a "cutoff" score for Step 1, below which applicants are unlikely to be considered, although in some cases individuals with higher CK Stage 2 scores may still receive further consideration. The NRMP Housing Program Director's Survey contains more information, either overall or specifically, on "cutoff" scores (ie, scores below which programs generally do not provide interviews).
